Dear Child Care Provider,

This is to inform you that the Department of Children and Family Services (DCFS) has mailed letters verifying the average number of children served for purposes of claiming the School Readiness Tax Credit (SRTC) to all centers that were rated TWO - FIVE STARS for the tax year. If you have more than one qualifying center, each center will receive a separate letter.

For questions, please note the following:

• If you have questions about ELIGIBILITY FOR the staff member SRTC, you may call LA PATHWAYS at (318) 677-3163 or (800) 245-8925.
• For questions about ELIGIBILITY FOR the center SRTC, you may call (225) 342-0694 or (800) 680-9098.
• If you have questions about HOW TO ENTER THIS TAX CREDIT ON YOUR TAX RETURN, you must call the LA DEPT OF REVENUE at (225) 219-0102.
• WHERE IS MY REFUND? For questions about your REFUND and when it will be received, you must call LA DEPT OF REVENUE at (225) 219-0102.
